Free Webinar on HHP

On March 30, join us to learn the basics of California's new Health Homes Program (HHP) for Patients with Complex Needs.

The state of California is moving forward with implementation of the HHP program, providing extensive care coordination for the highest-risk Medi-Cal beneficiaries, to improve their health outcomes. Implementation is scheduled to begin in certain counties starting January 1, 2017.

Justice in Aging is providing a free webinar training series to inform and educate advocates of the upcoming changes and help them understand how their clients will be impacted by this new program. The first webinar will cover:

  • What is the HHP?
  • Who is affected by the HHP?
  • When and where is the HHP being implemented?
  • How does the HHP interact with other initiatives in California aimed at providing care coordination?

The webinar will take place on Wednesday, March 30, 2016 11 a.m. - 12 p.m. PDT. Click here to register.
