Community Based Organizations and Health Care Partnerships

Through support from the Robert Wood Johnson Foundation, the Center for Health Care Strategies (CHCS) is collaborating with the Nonprofit Finance Fund and the Alliance for Strong Families and Communities to identify and disseminate best practices for partnership models between community-based organizations (CBOs) and health care systems, especially those that serve low-income and vulnerable populations. As you are well aware, it is increasingly clear that CBOs play a valuable role in helping patients access much-needed social services.
We are inviting partners to share this request for information (RFI) survey with relevant organizations that have formed such partnerships, to help advance what we know about these critical relationships. The survey is designed to: (1) identify promising models and challenges for building effective partnerships; (2) inform funding and policies; and (3) advance effective partnerships across the nation. Ideally the survey will uncover a wide variety of partnership models between non-profit and for-profit CBOs (e.g., housing organizations, workforce development agencies, food banks, early childhood education providers) and health care systems (e.g., health systems, hospitals, providers, and insurers, state and local public health entities), so we encourage a broad array of organizations to complete the questionnaire – feel free to share this with any relevant organizations.                  
Key RFI findings will be synthesized and shared widely through publications, webinars, and other. Using RFI responses, we will select up to five partnerships to showcase nationally as case studies.
We encourage you to share this with your partners who are engaged in such work and can contribute to an emerging knowledge base.  Please complete the RFI by Friday, January 27, 2017.  We'll be posting a blog on this next week, so if its easiest to perhaps tweet this to your followers as a way of promoting this among interested partners, let us know and we'll give you a heads up on this!

Top 5 Reasons for Choosing Adult Day Services


The U.S.population over the age of 65 has grown 3 times faster than the population under 65, with a 30-year life-expectancy increase in the past 100 years. We are living longer, but not necessarily healthier. Risks of suffering from chronic, limiting conditions such as arthritis, diabetes, osteoporosis, and dementia increase with age. As more people live into their 80s and 90s, more people require custodial care, supervision, and assistance performing activities of daily living.
Nearly half of our six million adults over age 85 need assistance performing everyday activities such as keeping the house, preparing meals, bathing, and getting around in and outside the home. In response to their gradual loss of independence, family members step up and become caregivers, assuming responsibility for guarding their health, safety, and dignity. Family caregivers (unpaid) provide 83% percent of the help provided to older adults. They also help mitigate other factors associated with increased dependency, such as loss of social outlets, loneliness, depression, and apathy. Many seniors have in their closest relative not only their sole caregiver but also their single source of social contact. CONTINUE READING
