Through support from the Robert Wood Johnson Foundation, the Center for Health Care Strategies (CHCS) is collaborating with the Nonprofit Finance Fund and the Alliance for Strong Families and Communities to identify and disseminate best practices for partnership models between community-based organizations (CBOs) and health care systems, especially those that serve low-income and vulnerable populations. As you are well aware, it is increasingly clear that CBOs play a valuable role in helping patients access much-needed social services.
We are inviting partners to share this request for information (RFI) survey with relevant organizations that have formed such partnerships, to help advance what we know about these critical relationships. The survey is designed to: (1) identify promising models and challenges for building effective partnerships; (2) inform funding and policies; and (3) advance effective partnerships across the nation. Ideally the survey will uncover a wide variety of partnership models between non-profit and for-profit CBOs (e.g., housing organizations, workforce development agencies, food banks, early childhood education providers) and health care systems (e.g., health systems, hospitals, providers, and insurers, state and local public health entities), so we encourage a broad array of organizations to complete the questionnaire – feel free to share this with any relevant organizations.
Key RFI findings will be synthesized and shared widely through publications, webinars, and other. Using RFI responses, we will select up to five partnerships to showcase nationally as case studies.
We encourage you to share this with your partners who are engaged in such work and can contribute to an emerging knowledge base. Please complete the RFI by Friday, January 27, 2017.
