Community Partnership Grants available


Cottage Health has released the guidelines for the 2016 Community Partnership Grants, which will focus on access to health services.  The full text of the guidelines appears below the jump below. 2016 Community Partnership Grant Guidelines

Cottage Health is proud to be able to support non‐profits in our community through our community partnership grant program. In response to a 2013 Community Health Needs Assessment, which is available at www.cottageheatlh.org, grants awarded for 2015, 2016, and 2017 will be for Access to Health Services using ideology from Healthy People. Our next Community Health Needs Assessment will occur in 2016. Those results will be shared with community organizations and made publicly available on our website.

Access to comprehensive, quality health care services is important for the achievement of health equity and for increasing the quality of a healthy life for everyone. It impacts: overall physical, social, and mental health status; prevention of disease and disability; detection and treatment of health conditions; quality of life; preventable death; and life expectancy.

Access to health services means the timely use of personal health services to achieve the best health outcomes. It encompasses three distinct steps: 1) Gaining entry into the health care system; 2) Accessing a health care location where needed services are provided; and 3) Finding a health care provider with whom the patient can communicate and trust.

To make an impact, the annual competitive grant process will result in funding a small number of programs at a higher dollar amount, with grants typically between $20,000 and $100,000 per program. Any organization registered as a 501(c)3 with the Internal Revenue Service or government agency/school serving South and/or Mid Santa Barbara County may request funding to help support its organization in terms of program expenses, general operating costs, and capital expenditures.

The first step in the grant process is to complete a Letter of Inquiry (LOI), which should include the following information about the program:
  1. description,
  2. how it provides access to health services as described in at least one of the three steps above,
  3. total budget,
  4. amount requesting,
  5. funding sources,
  6. number of unique people served,
  7. measurable outcomes, and
  8. transformational goals/aspirations/vision.
Organizations requesting funding should submit a separate LOI for each program seeking support. The LOI should be no more than two pages long in a 12‐point font with 1‐inch margins. Email the completed LOI on letterhead to mray@sbch.org.

Key dates for 2016 partnership:

  • Letter of Inquiry due by 5:00 pm on Monday, August 31, 2015
  • Partnership application invitations announced in early October 2015
  • Partnership applications due in late October 2015
  • Partners announced in December 2015
  • Partnership term is January 1, 2016 through December 31, 2016
